This patient is a 20-year-old female who desired having a bilateral breast augmentation. She expressed the frustration of tops, dresses, swimsuits, and even bras not fitting properly. She was a petite, slender woman who had a small amount of asymmetry with the right breast just slightly larger than the left breast. She had a 34A breast size and desired a C-cup breast size. We reviewed the various approaches for a breast augmentation including transaxillary, periareolar, and inframammary approaches. In addition, we reviewed in detail the type and positioning of the implants. We discussed subglandular versus submuscular as well as the duo plane approach. We decided together that the best approach for her breast surgery would be the duo plane approach with the incisions made around the areola. The cosmetic breast surgery was performed by Dr. R. Michael Koch under MAC anesthesia in our board certified surgical suite in Tarrytown, New York. After surgery she went on a shopping spree and was very happy to have cleavage and fuller breasts to fill out her new clothing.
