Your procedure begins in coordination with your breast or general surgeon, who performs a skin-sparing or nipple-sparing mastectomy to preserve as much native breast skin as possible. Immediately after the mastectomy, Dr. Jacobs initiates the reconstruction by elevating the pectoralis major muscle from the chest wall. He then sutures a sterile, processed sheet of acellular dermal matrix (ADM) to the lower edge of the pectoralis major muscle.
This ADM acts as an extension of the muscle, and its lower edge is anchored to the inframammary fold to form a complete submuscular pocket. A silicone or saline implant is placed into this pocket for immediate volume and shape.
